You are invited to participate in an ongoing research project that is looking for risk factors that might contribute to the development of motor neurone disease (MND). An online anonymous questionnaire asks a series of questions that may provide clues as to what factors trigger MND in some people. Anyone is welcome to complete the questionnaire - people with a sporadic or familial form of MND, their relatives and also people without MND in order to compare lifestyle factors. This questionnaire can be completed regardless of geographical location, so please share with any friends or family who may live overseas who would be happy to contribute.

The questionnaire, designed by Dr Roger Pamphlett at the University of Sydney, and instructions for how to complete it can be found at www.alsquest.org. This study has been approved by the Ethics Review Committee (RPAH Zone) of the Sydney Local Health District. Please note, if you have previously submitted a response to the ALS Quest questionnaire, there is no need to complete it again.
