Multidisciplinary care is a term used when a group of different health and community care professionals work together to provide treatment and support. 

Your MND Advisor is part of the multidisciplinary care team and can support you to access to the following professionals as required.

Case Manager/Care Coordinator

For coordinating level 3 or 4 home care package funding from My Aged Care (MAC).

Dietitian

For dietary and nutritional advice.

Gastroenterologist

For tube feeding. GP referral required.

General Practitioner (GP)

General medical care, referrals and medications.

Neurologist

The diagnosing specialist who monitors disease progression. GP referral required.

NDIS Support Coordinator

NDIS plan implementation for people with MND who are accessing for the NDIS.

Occupational Therapist

For advice about mobility, function and everyday tasks, including equipment prescription and home modifications.

Palliative Care

For interventions to improve quality of life, emotional support and to support future planning. GP referral generally required.

Physiotherapist

For advice about pain management, physical activity, mobility, stretching, breathing and positioning.

Psychologist

For support with emotional well-being and the development of strategies to continue to participate in the community. GP referral required.

Registered Nurse, Clinical Nurse

Ongoing clinical care and coordination.

Respiratory Specialist

For respiratory changes and prescribing non-invasive ventilation (NIV). GP referral required.

Speech Pathologist/Therapist

For the management of communication, swallowing and saliva management.

Social Worker

For general counselling and to provide information about community services.
